• Step 1: The first step is to make sure you have a copy of the Cause Mapping template in Excel. If you don't, you can download a copy using the button below.

• Step 2: Next, you'll need to build a Cause Map diagram of the incident using the template in Excel. 

• Step 3: Submit your completed Cause Map file using the form to the right.
